The first stitch of Row 5 should be a yarn over.

Add a column of stitches to the left side of the chart. This will add 1 extra knit stitch to the end of every RS row and 1 extra purl stitch at the start of every WS row, making the chart symmetrical and the correct width of 21 stitches.
